METHODOLOGY: Survey conducted by AIS researchers and supplemented by publicly available information sources. Tiers are as defined by each plan. Average copay per tier is calculated by each company based on their entire book of PBM business. Calculation method not specified by AIS. Company averages as reported to AIS are then used to create an industry-wide average for each tier copay.
