METHODOLOGY:
Calculated from enrollment data for all types of health plans (commercial,
Medicare, Medicaid, etc.), offered by all U.S. health insurers that sell fully
insured products. Data was compiled from health plan surveys conducted by AIS
researchers, supplemented and verified by state and federal government data
where available, during the fourth quarter of each year indicated, and
originally published in AIS’s
